RN/LPN
Assist with the care and needs of our residents in our Skilled Nursing, Long Term Care, Assisted Living or Assisted Living Memory Care Support Facilitates.
Qualifications
•A current, active LPN
•CPR or BLS certification
Responsibilities
•Schedules will include hours on the weekend and some holidays
•Provide basic bedside care to long term care, skilled nursing and assisted living residents under the direction of a Registered Nurse (RN) or physician, functioning within scope of license
•Contribute to the assessment of residents by collecting data for analysis by a Registered Nurse (RN)
•Administer medications and observe residents for adverse reactions to medications or treatments
•Perform routine laboratory tests and simple therapeutic services
•Educate residents and their families about disease treatment plan including self-care post discharge, holistic health needs, available resources and follow up care
•Assure resident safety by identifying safety hazards and emergency situations and initiates corrective action as appropriate
CNA
Qualifications
•State certification as a nurse aide is required
•Complete familiarity with duties of a certified nurse aide
•Highly developed observation skills
•Excellent communication and customer service skills
•Must be legally authorized to work in the United States
Responsibilities include, but not limited to:
•Schedule: Mulitple Shifts
•Schedules with guaranteed hours will include hours every other weekend and some holidays
•Assist with procedures ordered by physician and supervised by a Registered Nurse (RN)
•Assist patients with tending to personal care, activities of daily living and transfers/transport
•Report findings or changes in physical, mental and emotional conditions to nursing staff
•Assist with keeping patient rooms clean and orderly
•Assure resident safety by identifying safety hazards and emergency situations and initiates corrective action as appropriate
CMA
Qualifications
•Certified Medication Aide (CMA) Qualifications
•State certification as a nurse aide is required
•State approved medication administration certification
•Complete familiarity with duties of a certified nurse aide
•Highly developed observation skills
•Excellent communication and customer service skills
Responsibilities
•These tasks are performed as ordered by a physician, under the supervision of a licensed nurse, in accordance with state and federal rules and regulations, and consistent with facility policies
•Be a part of a multi-disciplinary team that is recognized for their achievements and contributions
•Must demonstrate knowledge of the “five rights” of medication administration
•Records the administration of medications and/or treatments appropriately
•Observes for and reports any reactions and/or side effects of medications to a licensed nurse on the appropriate shift
•Administers PRN medications in accordance with state and federal regulations and consistent with facility policy
Dietary
Qualifications
•Must be able to lift up to fifty pounds
•Must be able to work as a team member
•Solid work ethic, with an initiative for quality and customer service
•Minimum of high school diploma or equivalent
•Must be reliable & keep to schedule
•Must be legally authorized to work in the United States
•Must possess a strong desire to work with the elderly
•Must have the ability to multitask
The Dietary Aide is responsible for serving food in accordance with federal, state, and local standards, guidelines, and regulations with Heritage Community Dunaway Manor established policies and procedures. Job duties include, but are not limited to:
• Set up tables or other dining areas with attention to cleanliness and order
• Prepare meals under the supervision of a Dietary Manager or Dietary Cook
• Deliver dishes and trays at designated times and rooms
• Take down dining areas and collect plates, glasses, etc.
• Discard leftovers and take out the garbage
• Clean food stations and wash dishes, cooking vessels and silverware
• Stock ingredients and monitor inventory
• Assist in keeping track of expenses and menu planning
• Help maintain kitchen equipment and appliances
Housekeeping
Qualifications
•Must be able to lift up to fifty pounds

•Maintains a high level of cleanliness & safety throughout designated areas
•Ensure that all equipment is clean and in working condition at all times
•Removal of trash & soiled linens
